We collect information from you when you fill out our contact form and appointment request form.When writing on the contact form on our site, as appropriate, you will be asked to enter your name, e-mail, and phone number so we can contact you. From our appointment request form, we also ask for your name, birth date, address, phone number, email insurance, and insurance id, in order to call you and set you up for an appointment.

Your information, whether public or private, will not be sold, exchanged, transferred, or given to any other company for any reason whatsoever, without your consent. Our request form will store information in our appointment system so we can schedule an appointment.

We implement a variety of security measures to maintain the safety of your personal information. We delete data that we collect from our website every six months.
